Quick answer

A semaglutide tracker logs your dose, injection site or pill timing, weight trend, protein & side effects across Ozempic, Wegovy & Rybelsus. Here's what to track.

Semaglutide is a single GLP-1 receptor agonist — one molecule — sold under three brand names: Ozempic (weekly injection for type 2 diabetes), Wegovy (weekly injection for chronic weight management), and Rybelsus (a daily oral tablet for type 2 diabetes).

It is not the same drug as tirzepatide (Mounjaro, Zepbound), which is a different, dual-hormone molecule.

Because the same active ingredient comes in different doses, routes, and schedules, the one thing every semaglutide plan rewards is clean record-keeping.

This guide covers what to track on semaglutide, how tracking differs between the injectable and oral forms, and how brand versus compounded semaglutide changes the picture.

It is neutral and educational — you can do all of it on paper.

If you already know your specific brand, jump to the brand-specific guides: our Ozempic tracker guide and Wegovy tracker guide.

If you want the general framework across all GLP-1s first, start with the complete GLP-1 tracker app guide.

01

What is semaglutide? One molecule, three brands

Semaglutide mimics the gut hormone GLP-1, which slows gastric emptying and acts on appetite centers in the brain (Cleveland Clinic).

The confusion most people hit is that the same molecule is marketed under three names with different indications, doses, and schedules.

Here is the map — and it is exactly why a tracker matters.

(Doses per the FDA labels — see References. Off-label prescribing happens; always follow your own prescriber and label.)

The takeaway: whichever brand you are on, you are on semaglutide — but the schedule you must track is different for each. That is what the rest of this guide unpacks.

BrandForm & scheduleFDA-approved useDose range
OzempicOnce-weekly injectionType 2 diabetes0.25 → 0.5 → 1 → up to 2 mg
WegovyOnce-weekly injectionChronic weight management0.25 → 0.5 → 1 → 1.7 → 2.4 mg; up to 7.2 mg (approved 2026)
RybelsusDaily oral tabletType 2 diabetesR1: 3 → 7 → 14 mg; R2: 1.5 → 4 → 9 mg

02

Why tracking matters on semaglutide specifically

Three features of semaglutide make tracking more than a nice-to-have:

The dose escalates on a schedule.

Every semaglutide brand starts low and steps up over weeks to reduce gastrointestinal side effects. Logging which dose you took on which date keeps you synced to that plan and gives your prescriber an exact history at adjustment visits.

The timing is easy to miss.

A weekly injection is easy to forget or delay; a daily pill has strict fasting rules that are easy to break. Either way, a timestamped log turns a stressful "did I?" into a glance.

Results and side effects are non-linear.

Weight comes off in fits and plateaus, and nausea tends to spike right after each dose increase. Only a trend — not a single day — shows what is actually happening.

03

What to track on semaglutide (and why each one matters)

Here is the core semaglutide dataset. Use it as your checklist whether you inject (Ozempic/Wegovy) or take the oral tablet (Rybelsus), on paper, in a spreadsheet, or in an app.

What to trackWhy it mattersHow often
Dose date & amount (mg)Keeps you aligned with the titration schedule; confirms adherence; prevents accidental double-dosingEvery dose (weekly injection / daily pill)
Injection site & rotation (injectable)Rotating sites prevents lumpy scar tissue (lipohypertrophy) that changes drug absorptionEvery injection
Pill timing & fasting window (oral)Rybelsus only absorbs correctly on an empty stomach with the right water and waitEvery morning dose
Body weightThe primary outcome; the trend reveals plateaus and metabolic adaptation1–3× per week, same time of day
Waist / measurementsCatches fat loss the scale misses, especially while preserving muscleEvery 1–2 weeks
Protein intake (grams)Protects lean muscle mass, which is at risk during rapid weight lossDaily
Water / hydrationEases the constipation and nausea common in the first weeksDaily
Side effects & severitySeparates normal adaptation from red-flag symptoms needing a doctorAs they occur
Blood sugar (if diabetic)Ozempic and Rybelsus are diabetes medicines; glucose is a core outcomePer your care plan
Refill / supplyA weekly pen or a daily pill pack runs out on a predictable date; avoids a coverage gapMonthly

04

Injectable vs oral semaglutide: what changes about tracking

This is the fork that trips people up. The active ingredient is identical, but what you record depends on whether you inject or swallow it.

Injectable semaglutide (Ozempic & Wegovy)

Both injectable brands are given subcutaneously into the abdomen (at least two inches from the navel), the front of the thigh, or the upper arm, rotating each week (wegovy.com pen guide; ozempic.com dosing).

Repeatedly injecting the same spot can cause lipohypertrophy — lumpy scar tissue that changes absorption.

Track which mg you took, on what date, at which site. Their titration schedules differ:

  • Ozempic steps 0.25 mg (weeks 1–4, a starting dose, not for glycemic control) → 0.5 mg → 1 mg → up to a 2 mg maximum, as your prescriber directs (FDA Ozempic label).
  • Wegovy steps 0.25 → 0.5 → 1 → 1.7 → 2.4 mg maintenance (novoMEDLINK Wegovy dosing). 2.4 mg weekly is the standard maintenance dose. A higher 7.2 mg weekly maintenance dose was approved in the EU (February 2026) and the US (March 2026, as Wegovy HD) for adults with obesity who have taken 2.4 mg for at least 4 weeks and need more weight loss; altered skin sensations (dysesthesia) were much more common at 7.2 mg (21.6% vs 0.3% on placebo in the STEP UP trial).

Because the two brands top out at different doses, a dose log removes any doubt about where you are in your specific schedule.

See our Ozempic tracker and Wegovy tracker for the brand-by-brand detail.

Oral semaglutide (Rybelsus)

Rybelsus is a different tracking problem: it is a once-daily tablet with strict timing rules. Per Novo Nordisk and the FDA label, take it first thing in the morning on an empty stomach, with no more than 120 mL (about 4 oz, half a glass) of plain water, then wait at least 30 minutes before eating, drinking anything else, or taking other medicines (novoMEDLINK Rybelsus dosing; FDA Rybelsus label).

Swallow the tablet whole — do not split, crush, or chew. Its titration is 3 mg for 30 days → 7 mg → 14 mg (formulation R1).

Rybelsus (oral semaglutide for type 2 diabetes) comes in two formulations: R1 (3/7/14 mg) and R2 (1.5/4/9 mg).

Because 1.5, 4 and 9 mg are also Wegovy-pill escalation steps, the mg alone does not tell the products apart — log the brand name, not just the mg.

For oral users, the tracker's job is a daily reminder plus the fasting window, not an injection-site map — because breaking the empty-stomach rule quietly cuts absorption.

05

Missed-dose rules differ by brand — do not mix them up

This is the single most confused semaglutide fact, and getting it wrong on the wrong brand matters. Log every dose with a timestamp so the decision is a glance, not a guess.

BrandMissed-dose rule
Wegovy (weekly)If the next dose is more than 2 days (48 h) away, take the missed dose ASAP; if less than 2 days away, skip it and resume on schedule (US FDA label; EU leaflets: within 5 days of the missed dose)
Ozempic (weekly)Take the missed dose ASAP within 5 days; if more than 5 days have passed, skip it and resume on schedule (don't take two doses within 48 h)
Rybelsus (daily)Skip the missed dose entirely and take the next dose the following day — do not double up

Sources: novoMEDLINK Wegovy dosing; FDA Ozempic label; FDA Rybelsus label.

The US labels word Wegovy's and Ozempic's rules differently ("next dose more than 2 days away" vs "within 5 days of the missed dose"), but for a weekly shot they describe the same window.

This is the US FDA label wording.

The EU and many other countries' Wegovy leaflets say: take the missed dose within 5 days of the day you missed, otherwise skip it — check the leaflet in your own country and your prescriber.

Always follow your own label and prescriber — this is education, not direction.

06

Brand vs compounded semaglutide: what to track and know

You may have seen "compounded semaglutide" offered by telehealth clinics or med-spas, often cheaper than brand-name pens.

Here is the regulatory reality as of 2026, because it affects what you are actually injecting.

  • Brand-name semaglutide (Ozempic, Wegovy, Rybelsus) is FDA-approved — reviewed for safety, efficacy, and manufacturing quality.
  • Compounded semaglutide is not FDA-approved. Compounding was widely allowed only while semaglutide was on the FDA drug-shortage list. The FDA declared that shortage resolved on February 21, 2025, and set enforcement deadlines: 503A pharmacies until April 22, 2025 and 503B outsourcing facilities until May 22, 2025 (FDA compounding policy statement).
  • After those dates, routine large-scale compounding of copies of approved semaglutide is no longer permitted; only narrow patient-specific 503A exceptions (e.g., a documented excipient allergy, or a strength not commercially available) may remain.

If you are on a compounded product, track the exact source, concentration, and dose in your own units carefully — compounded formulations can differ in strength and units from brand pens, which is a documented dosing-error risk.

When in doubt, ask a licensed pharmacist or your prescriber, and prefer FDA-approved products.

07

Semaglutide vs tirzepatide: not the same molecule

A common mix-up: semaglutide is a single GLP-1 receptor agonist, while tirzepatide (Mounjaro, Zepbound) is a dual GIP + GLP-1 receptor agonist — a different molecule that acts on two hormone pathways and, in head-to-head trials, produced greater average weight loss.

If you are deciding between them, or switching, that is a separate conversation covered in our semaglutide vs tirzepatide guide.

For tracking purposes, the two are not interchangeable: never log a tirzepatide dose against a semaglutide schedule.

Safety basics for any GLP-1 log. Don't use these medicines if you or a close relative has had medullary thyroid cancer, or if you have MEN 2.

They are not used during pregnancy — tell your prescriber right away if you become pregnant or plan to, because some must be stopped weeks to months beforehand.

If you also take insulin or a sulfonylurea, watch for low blood sugar (shakiness, sweating, confusion).

Vomiting or diarrhea that keeps you from holding down fluids can dehydrate you and strain your kidneys — call your clinician.

A tracker organizes your records; the package leaflet in your country and your prescriber come first.

08

How to build a semaglutide tracking routine in 6 steps

You do not need a complicated system — you need a repeatable one. The core log takes under two minutes a week (or one tap a morning for Rybelsus), plus quick daily taps.

  1. 1

Set a dose reminder that matches your form.

A recurring weekly alert for Ozempic/Wegovy, or a daily morning alarm for Rybelsus so the fasting window isn't missed.

  2. 2

Log the dose right after taking it.

Record the mg amount and — if injecting — mark the body-map zone so next week's rotation is obvious.

  3. 3

    Weigh in on fixed days.

Choose 1–3 mornings a week, same conditions, and read the trend — not the daily noise.

  4. 4

    Track protein and water daily.

A quick tap per meal keeps muscle protection on track.

  5. 5

Note side effects when they happen.

Record the symptom, severity, and days since your last dose or dose increase.

  6. 6

Photo every 2–4 weeks and review before each visit.

Same lighting, same angle — and bring your dose log and side-effect timeline to your clinician.

09

Do you need a semaglutide app, or is a notebook fine?

Honestly, any tracking beats none — a semaglutide diary in a notebook works.

What an app adds is the automation of the parts people forget: the dose reminder tuned to weekly-vs-daily, the injection-site rotation map for the pens, the trend line that smooths daily weight noise, and a side-effect timeline you can hand to your clinician.

None of the three brands ships with a dedicated companion app, so most people use a general GLP-1 or wellness tracker.

If you want your dose log living next to the daily habits that protect your results — protein, movement, hydration, weight and body-composition trends — a wellness app like HAVIT is a natural home for it.

Compare options in our best GLP-1 tracker apps guide and pick what you will actually keep using; consistency matters far more than the brand.

Why HAVIT — Medication + Behavior Change, in One App

GLP-1 medications can meaningfully reduce weight — but the number on the scale is only part of the story.

The lean-mass share of the weight lost on GLP-1 therapy has ranged from 40–60% in some trials to 15% or less in others — and lean mass counts organs, bone and fluid, not only muscle (Neeland et al., Diabetes, Obesity and Metabolism, 2024), and results tend to fade once the medication stops unless new habits take their place.

That's why leading guidance treats medication as one part of care, not the whole: the U.S. Preventive Services Task Force recommends that adults with obesity be offered intensive, multicomponent behavioral interventions (USPSTF, Grade B).

HAVIT is built for exactly this moment — one app that unites medication tracking, body composition, nutrition and protein, daily habit-building, and an AI coach that adapts to you.

Across our own 1,090 GLP-1 users, the most consistent trackers built the strongest habits (see the GLP-1 habit report).

Frequently asked questions

What is a semaglutide tracker?
It is any tool — app, spreadsheet, or notebook — that records the key parts of semaglutide treatment: your dose and date, the injection site (or pill timing for Rybelsus), weight, protein, water, side effects, and photos. It turns those into reminders and trend charts you can review and share with your doctor.
Are Ozempic, Wegovy, and Rybelsus the same drug?
Yes — all three contain the same active ingredient, semaglutide, a single GLP-1 receptor agonist. They differ by form and approval: Ozempic and Rybelsus are for type 2 diabetes (injection and oral tablet), while Wegovy is an injection for weight management, each with its own dose schedule.
How is tracking oral semaglutide (Rybelsus) different?
Rybelsus is a daily tablet with strict timing: take it first thing in the morning on an empty stomach with no more than 120 mL (about 4 oz) of water, then wait at least 30 minutes before anything else. So a Rybelsus tracker centers on a daily reminder and the fasting window, not an injection-site map.
What is the semaglutide dose schedule I should log?
It depends on the brand. Ozempic steps 0.25 → 0.5 → 1 → up to 2 mg weekly; Wegovy steps 0.25 → 0.5 → 1 → 1.7 → 2.4 mg weekly (a higher 7.2 mg maintenance dose was approved in 2026); Rybelsus steps 3 → 7 → 14 mg daily (formulation R1) or 1.5 → 4 → 9 mg (formulation R2) — log the brand, not just the mg. Log which dose you took on which date so your prescriber sees the full history.
What happens if I miss a semaglutide dose?
It varies by brand. Under the US labels Wegovy uses a 2-day rule and Ozempic a 5-day rule — worded differently but the same window for a weekly shot (EU Wegovy leaflets say within 5 days) — and Rybelsus says skip the missed dose and resume the next day. Never double up. This is exactly why a timestamped dose log is so useful — always follow your own label.
Is compounded semaglutide the same as the brand?
No. Brand semaglutide is FDA-approved; compounded versions are not. Wide compounding was allowed only during the FDA shortage, which was declared resolved on February 21, 2025, with 503A/503B deadlines in April–May 2025. If you use a compounded product, track its exact source, concentration, and units carefully.
Is semaglutide the same as tirzepatide?
No. Semaglutide is a single GLP-1 receptor agonist (Ozempic, Wegovy, Rybelsus). Tirzepatide (Mounjaro, Zepbound) is a different, dual GIP + GLP-1 molecule. They are not interchangeable, so never log a tirzepatide dose against a semaglutide schedule.
Is there an official semaglutide tracking app?
No brand of semaglutide comes with a dedicated companion tracking app. Most people use a general GLP-1 or wellness app, or a simple notebook, to log doses, sites or pill timing, weight, and side effects. Choose whichever you will keep using — that matters more than the specific tool.
