Quick answer

Tirzepatide (Mounjaro, Zepbound) acts on two hormones (GIP + GLP-1) and produced greater average weight loss in trials (~21%) than semaglutide (Ozempic, Wegovy, ~15%). But semaglutide has the longer track record and strong cardiovascular data. The "better" one depends on your goals, tolerance, cost, and prescriber.

Both drugs belong to the same broad family, but they are not the same molecule. Semaglutide is a single GLP-1 receptor agonist, sold as Ozempic and Rybelsus (type 2 diabetes) and Wegovy (weight management). Tirzepatide is a dual GIP/GLP-1 agonist, sold as Mounjaro (type 2 diabetes) and Zepbound (weight management).

That extra hormone target — GIP — is the headline difference and, in trials, translates into more average weight loss.

This page walks through what actually separates them so you can have a sharper conversation with your clinician.

02

How they work: single vs dual hormone

Semaglutide mimics one gut hormone, GLP-1, which slows gastric emptying and acts on brain appetite centers so you feel full sooner and longer.

Tirzepatide does that and activates the GIP receptor — a second incretin hormone — which is thought to add to appetite and metabolic effects.

Both mechanisms are described by the FDA labels and major clinics such as Mayo Clinic.

The shared GLP-1 action is why their side-effect profiles overlap so heavily.

03

Weight loss: what the trials show

The cleanest way to compare is the trials. In STEP-1, semaglutide 2.4 mg produced a mean 14.9% weight loss over 68 weeks.

In SURMOUNT-1, tirzepatide produced 16.0% (5 mg), 21.4% (10 mg), and 22.5% (15 mg) over 72 weeks.

Crucially, these two were finally compared head-to-head in SURMOUNT-5 (NEJM, 2025): over 72 weeks in adults with obesity but without diabetes, tirzepatide reduced body weight by −20.2% versus −13.7% for semaglutide (p<0.001).

On efficacy alone, tirzepatide won — but "more weight loss" is only one axis of a good decision.

04

Side effects and tolerability: are they different?

They share the same broad GI profile — nausea, vomiting, diarrhea, constipation — because they act on the same GLP-1 pathway.

A common assumption is that the stronger drug must be harder to tolerate, but SURMOUNT-5 found the opposite direction: GI adverse events causing discontinuation were slightly more frequent with semaglutide (5.6%) than tirzepatide (2.7%). Both carry the FDA boxed warning for thyroid C-cell tumors and the same serious risks (pancreatitis, gallbladder disease).

The biggest predictor of how you'll tolerate either drug is how quickly you escalate the dose, not which brand you pick.

For the full breakdown, see our tirzepatide side effects guide.

05

Dosing and forms

Semaglutide (Wegovy) titrates 0.25 → 0.5 → 1.0 → 1.7 → 2.4 mg in four-week steps; tirzepatide titrates 2.5 → 5 → 7.5 → 10 → 12.5 → 15 mg in four-week steps, per the FDA labels.

One practical differentiator: semaglutide has an oral option (Rybelsus) for type 2 diabetes, while tirzepatide is injection only. Your first four weeks look similar on either — see our GLP-1 first-month guide.

06

Cost and access

Both are expensive without coverage, and pricing shifts frequently, so verify current numbers with your pharmacy and plan.

Access also differs by indication: Ozempic and Mounjaro are FDA-approved for type 2 diabetes, while Wegovy and Zepbound are approved for weight management — which drug your insurance will cover often hinges on that distinction and your diagnosis.

Compounded versions are no longer broadly permitted now that the shortages have resolved; only the FDA-approved brands are quality-assured.

07

Which should you choose?

Neither is universally "better." Weigh these, then decide with your prescriber:

  1. Maximum weight loss is the priority → the head-to-head data favor tirzepatide.
  2. You want the longest track record and established CV-benefit datasemaglutide leads here.
  3. You prefer a pill over an injection → only semaglutide offers oral (Rybelsus, for T2D).
  4. Tolerability / side-effect history → individual, and driven more by titration speed than by brand.
  5. Cost and insurance → coverage often follows your indication (diabetes vs obesity) and the specific brand on your formulary.
  6. Availability → confirm which brands and indications are approved and stocked in your region.

Why HAVIT — Medication + Behavior Change, in One App

GLP-1 medications can meaningfully reduce weight — but the number on the scale is only part of the story.

Roughly 20–40% of the weight lost on GLP-1 therapy can come from lean (muscle) tissue rather than fat (Neeland et al., Diabetes, Obesity and Metabolism, 2024), and results tend to fade once the medication stops unless new habits take their place.

That's why leading guidance treats medication as one part of care, not the whole: the U.S. Preventive Services Task Force recommends that adults with obesity be offered intensive, multicomponent behavioral interventions (USPSTF, Grade B).

HAVIT is built for exactly this moment — one app that unites medication tracking, body composition, nutrition and protein, daily habit-building, and an AI coach that adapts to you.

Across our own 1,090 GLP-1 users, the most consistent trackers built the strongest habits (see the GLP-1 habit report).

FAQ

Is tirzepatide better than semaglutide for weight loss?
On average, yes, for weight loss alone. In the head-to-head SURMOUNT-5 trial, tirzepatide reduced body weight by 20.2% versus 13.7% for semaglutide over 72 weeks. "Better for you," though, also depends on tolerance, cost, access, and your prescriber's judgment.
Are Ozempic and Mounjaro the same thing?
No. Ozempic is semaglutide (a single GLP-1 agonist); Mounjaro is tirzepatide (a dual GIP/GLP-1 agonist). Both are approved for type 2 diabetes, but they are different molecules with different average weight-loss results.
Which has fewer side effects?
They share the same GI side-effect profile. In SURMOUNT-5, discontinuation due to GI side effects was slightly higher with semaglutide (5.6%) than tirzepatide (2.7%). Tolerability is highly individual and driven mostly by how fast the dose is escalated.
Can you switch from semaglutide to tirzepatide?
Some people do, but switching is a medical decision your prescriber manages — including timing and the starting dose of the new drug. Do not switch or overlap on your own; ask your clinician about the right protocol.
Which works faster?
Both use a slow, multi-week titration and build effect gradually; neither is an overnight result. Appetite changes often appear within the first weeks, but meaningful weight loss accrues over months on both drugs.
Is tirzepatide more expensive than semaglutide?
Both are costly without insurance and prices change often. Which is cheaper for you depends on your plan, the specific brand, and your indication — check current pricing with your pharmacy rather than relying on a fixed figure.
Can you take semaglutide and tirzepatide together?
No. They act on the same GLP-1 pathway and are not meant to be combined; doing so raises side-effect risk without an established benefit. Use only one GLP-1-based drug at a time, as prescribed.
Which is right if I have type 2 diabetes vs weight loss only?
For diabetes, Ozempic (semaglutide) and Mounjaro (tirzepatide) are the approved options; for weight management it's Wegovy (semaglutide) and Zepbound (tirzepatide). Your diagnosis, other conditions, and insurance coverage guide the choice with your clinician.
