Tirzepatide does that and activates the GIP receptor — a second incretin hormone — which is thought to add to appetite and metabolic effects.

Weight loss: what the trials show

The cleanest way to compare is the trials. In STEP-1, semaglutide 2.4 mg produced a mean 14.9% weight loss over 68 weeks.

In SURMOUNT-1, tirzepatide produced 16.0% (5 mg), 21.4% (10 mg), and 22.5% (15 mg) over 72 weeks.

Crucially, these two were finally compared head-to-head in SURMOUNT-5 (NEJM, 2025): over 72 weeks in adults with obesity but without diabetes, tirzepatide reduced body weight by −20.2% versus −13.7% for semaglutide (p<0.001).

On efficacy alone, tirzepatide won — but "more weight loss" is only one axis of a good decision.

Side effects and tolerability: are they different?

They share the same broad GI profile — nausea, vomiting, diarrhea, constipation — because they act on the same GLP-1 pathway.

A common assumption is that the stronger drug must be harder to tolerate, but SURMOUNT-5 found the opposite direction: GI adverse events causing discontinuation were slightly more frequent with semaglutide (5.6%) than tirzepatide (2.7%). Both carry the FDA boxed warning for thyroid C-cell tumors and the same serious risks (pancreatitis, gallbladder disease).

Semaglutide (Wegovy) titrates 0.25 → 0.5 → 1.0 → 1.7 → 2.4 mg in four-week steps; tirzepatide titrates 2.5 → 5 → 7.5 → 10 → 12.5 → 15 mg in four-week steps, per the FDA labels.

Cost and access

Both are expensive without coverage, and pricing shifts frequently, so verify current numbers with your pharmacy and plan.

Access also differs by indication: Ozempic and Mounjaro are FDA-approved for type 2 diabetes, while Wegovy and Zepbound are approved for weight management — which drug your insurance will cover often hinges on that distinction and your diagnosis.
